Chin (submental) liposuction is a minimally invasive procedure that takes less than an hour, typically under local anesthesia (no IV or anesthesiologist needed). The patient is first given pre-medication before entering the operating room to make them more relaxed. Once the procedure begins, Dr.Rapaport injects a numbing agent and makes 2-4 small incisions below the ear and chin. From there, he injects tumescent solution, a numbing fluid which also constricts blood vessels, through these incisions. After that, the patient will no longer feel a thing during the procedure! Dr. Rapaport uses small cannulas, 3 millimeter, and less in diameter, to meticulously suction out all excess fat. Depending on the patient anatomy and plan, he may also perform micro liposuction to the jowls. Those are the cases which have four entry points rather than three because there would be an entry point under each jowl rather than a single midline entry point under the chin when the jowls are not being treated.

In many cases, immediately after the fat is suctioned out, the same entry points are used to deliver heat for tightening of the tissues which have now been made looser due to fat removal. The most common technology Dr Rapaport uses to achieve this tightening effect is called Renuvion. Renuvion uses a helium plasma which is ignited via RF stimulation to produce flashes of heat which cause collagen formation and therefore tightening.  After the procedure is completed, Dr. Rapaport often recommends patients undergo a session of Sofwave, to be done at a separate date, as early as two weeks after the liposuction. Sofwave delivers the final component of the trifecta, as we call it, by tightening the skin itself, as opposed to the wad of tissue from which the fat has been removed which the Renuvion takes care of.

Patients wear a headwrap for 48 hours to minimize bruising and swelling and maximize skin tightness following the procedure, and are typically back to work and normal activities within three days, making this a perfect procedure to do on a Friday. We offer a Motrin-like medication to patients following the procedure, but no narcotics or other controlled substances are needed during recovery.

Submental liposuction usually requires very little downtime; if a patient receives treatment on Friday, they are back to work on Monday. Patients typically experience minimal swelling and bruising, which will decrease after 48 hours. During submental liposuction recovery, keep incisions clean, dry, and protected from trauma. 

As noted above, the overwhelming majority of patients can return to work and full physical activity as early as three days after their procedure. The only real restriction relates to not swimming until the tiny wounds are more healed, around 10 days.

Submental Liposuction vs. Kybella: Kybella is an injectable deoxycholic-acid treatment that reduces a double chin by breaking down fat cells. It’s slightly  less invasive than neck liposuction in that it doesn’t create scars when properly used. However, liposuction offers precision unmatched by any other technique in that it is literally a precise sculpting procedure. Kybella on the other hand comes with several negatives. Firstly, the injection experience with kybella is noted to be particularly painful. Kybella also comes with substantial swelling in the neck area that typically lasts two weeks. This is a real problem particularly for people trying so hard to look better in that area. Finally, and maybe the biggest negative, is that because Kybella can kill nerves if it touches them, precise sculpting of the jawline simply can not be done with Kybella because the nerve that pulls down the lower lip passes through that area and must be avoided. All these are reasons why kybella is not a particularly popular treatment for reducing submental fat. In almost all cases, liposuction is a better option, and if the patient wants to remain truly non-invasive, then CoolSculpting would be the way to go.

Submental Liposuction vs. Neck Lift: Unlike liposuction, a neck lift is designed to directly address sagging skin and platysma muscles in addition to excess fat. Typically, patients in their thirties and forties, and sometimes older, prefer the more minimally invasive liposuction procedure as part of their trifecta (internal tightening with renuvion + skin tightening with Sofwave). However patients with more substantial laxity or those who have less laxity but expect a super smooth and tight result may do best opting for a neck lift.

Neck liposuction can also be paired with other procedures and treatments to achieve more comprehensive results. Patients with less elastic skin may benefit from a combination of neck lipo and skin-tightening treatments like Renuvion, SoftWave, FaceTite, AccuTite, microneedling, RF microneedling, or Ellacor.
